Patient Referral Information

Below are steps to expedite your referral

  • Your referring provider will give you the specialist’s name, address and phone number OR they will be mailing you this information.
  • Once you have the phone number for the specialist, please call the number to schedule your appointment.
  • The office will NOT call you. NOTE: Some offices may not be able to schedule your appointment when you call because they want to make sure they have all the needed information to schedule you. In some cases, it might be best to wait 3-5 days before you call.

Once you receive your appointment, please make every effort to keep this appointment. If you need to cancel, please notify them at least 24 hours in advance, so they can offer your appointment to another patient.

Should I wait for the specialist to call me for an appointment?
No, please call the specialist you were referred to. If you do not have the specialist’s name/phone number, call the office that referred you to get it.

How long will I have to wait to receive an appointment?
Check with your specialist to see how far they are booking out.

What happens if I have to cancel my appointment?
If you cannot keep your scheduled appointment, please notify the practice at least 24 hours in advance so that they can offer your appointment time to another patient.

What if my referral is urgent?
Contact the specialist you were referred to and let them know you need an appointment for an urgent referral.
