Patient Referral Information
Below are steps to expedite your referral
- Your referring provider will give you the specialist’s name, address and phone number OR they will be mailing you this information.
- Once you have the phone number for the specialist, please call the number to schedule your appointment.
- The office will NOT call you. NOTE: Some offices may not be able to schedule your appointment when you call because they want to make sure they have all the needed information to schedule you. In some cases, it might be best to wait 3-5 days before you call.
Once you receive your appointment, please make every effort to keep this appointment. If you need to cancel, please notify them at least 24 hours in advance, so they can offer your appointment to another patient.
