Priority Care Center RN

We are looking for an RN who is a leader, who is ambitious and excited about change and value in healthcare delivery, and who takes a holistic approach to serving clients.

The Priority Care RN is responsible for both the care management of a designated panel of clients with chronic conditions and/or complex health needs and direct clinical care of primary care clients at the Priority Care Center. Priority Care nurses work with clients, their support network, their providers and other members of the Priority Care team to provide support, guidance, education and assistance navigating the healthcare system. The RN is part of a multidisciplinary team that focuses on self-efficacy, wellness, safety and value for the clients while maintaining timely effective communication with the client’s care circle.  The RN will see clients at the Center, in their homes, at the hospital, and at their healthcare provider visits as needed. Bilingual in English and Spanish a plus.

Medical Assistant/Health Coach – Priority Care Center

Priority Care Center is looking for a full time Medical Assistant/Health Coach to join our care team. Our Medical Assistants/Health Coaches are integrated members of the clinical care team and work collaboratively with diverse patient populations to engage them in their health and well being.  The Medical Assistant (MA)/Health Coach is a pivotal member of the PCC (Priority Care Center) interdisciplinary team. You will use your people skills and caring personality to assist patients with care coordination needs, help close gaps in care and support PCC advance practice and behavioral health providers, nurses and wellness coaches.

We are seeking motivated, friendly responsible people who are able to work in a dynamic, fast paced work environment. This position provides great exposure to all areas of the healthcare field. Punctuality and willingness to learn is a must. While experience is a plus, full training is available.

Basic Qualifications:
High school diploma or equivalent; MA certification preferred
Experience as a medical assistant in a physician office setting preferred
Team player with strong interpersonal skills and ability to build effective working relationships throughout all levels of the organization
Experience working in EMR systems preferred, eCW a plus
Experience with MS Office suite
Bilingual in English and Spanish a plus

This is a full time M-F 8am-5pm position.

IT Assistant Director

The Humboldt IPA and The North Coast Health Improvement and Information Network are looking for an experienced, team-orientated individual to join our team as the IT Assistant Director. This position will work closely with the IT Director to support the day to day operations of the department as well as working with staff and our customers to promote programs and services of the organizations. The right individual will possess a strong understanding of health IT and HIE as well as having excellent interpersonal skills.  This is a full-time benefited position. Salary DOE.
